About Us Attestation of Sworn Affidavit Power of Attorney

To comply with local regulations which do not allow large gatherings and require safe distancing, Consulate General of India in Milan has decided to accept applications for (1) Attestation of Commercial Documents and (2) Attestation of Italian Translation of Apostiled Indian documents by post only.

Application form should be submitted in Miscellaneous Application form (Click here for application form ), duly completed with one photograph pasted on it, and signed by the applicant. Please also submit clear copies of (1) the relevant pages of the valid Indian Passport (two copies), (2)Permesso d’Soggiorno, (3) Residence Proof (Carta d’Identita, any Utility bill, etc.) and original as well as copy of the document to be attested.

(1) Attestation of Commercial Documents (Attestation can be done if only the documents are apostilled from Prefettura)

Application form should be submitted in Miscellaneous Application form (Click here for application form), duly completed with one photograph pasted on it, and signed by the applicant. Please also submit the following:

1. A letter from the company, in English, authorizing the bearer of the documents to get them attested from the Indian Consulate,

2. Clear copies of (1) the relevant pages of the valid Passport or Carta d’Identita,

3. Commercial documents, in duplicate.

 

(2) Attestation of Italian translation Apostilled Indian Docuemtns (Translation Verification)

Application form should be submitted in Miscellaneous Application form (Click here for application form), duly completed with one photograph pasted on it, and signed by the applicant. Please also submit clear copies of (1) the relevant pages of the valid Indian Passport (two copies), (2)Permesso d’Soggiorno, (3) Residence Proof (Carta d’Identita, any Utility bill, etc.) are also required, and (4) Original as well as copy of the documents required for attestation, in duplicate. If the document is issued from India, the same should be attested from Ministry of External Affairs, New Delhi. If the document is issued by any Department of the Italian Government or private organization, the same should first be apostilled from Prefettura and thereafter, submitted along with translation in English.

 

Following services are currently suspended at the Consulate and can not be availed by Postal modes, applicants seeking these services are requested to wait for further updates

(1) Power of Attorney

Application form should be submitted in Miscellaneous Application form (Click here for application form), duly completed with one photograph pasted on it, and signed by the applicant. Please also submit clear copies of (1) the relevant pages of the valid Indian Passport (two copies), (2)Permesso d’Soggiorno, (3) Residence Proof (Carta d’Identita, any Utility bill, etc.) are also required, and (4) Power of Attorney, in duplicate, with a passport size photograph pasted on the original as well as on the copy, (5) one clear copy of the Indian Passport or OCI card should be attached with the original Power of Attorney.

(2) Affidavit :

Affidavit for applying for passport in respect of minor children in India (As per new rules, if either of the parent is in India, there is no need for an Affidavit to get attested from Indian Missions/ Posts abroad. The parent, who submits application for passport in respect of minor children, can give an Affidavit in India itself). If however, the parent, who is in a foreign country, wants to give an Affidavit, giving his/her consent, the following are required:

Application form should be submitted in Miscellaneous Application form (Click here for application form), duly completed with one photograph pasted on it, and signed by the applicant.

Please also submit:

1. Affidavit(Click here for application form) if one of the parents is in the jurisdiction of this Consulate,

OR

Affidavit (Click here for application form) if both the parents are in the jurisdiction of this Consulate and they want to authorize their relative or friend to apply for passport in respect of their minor children,

2. Clear copies of (1) the relevant pages of the valid Indian Passport (two copies) of the applicant(s), (2)Permesso d’Soggiorno, (3) Residence Proof (Carta d’Identita, any Utility bill, etc.), (4) copies of the relevant pages of the passport(s) of the children, if applying for renewal or copy of the birth certificate of the children, if applying, afresh.
