About Us Life Certificate, Power of Attorney and Affidavit for minors passport in India

A. Who can apply:

Indian Nationals living in the areas of Lombardy, Piedmont, Aosta Valley, Trentino-Alto Adige, Friuli Venezia Giulia, Veneto, Liguria and Emilia-Romagna.

B. Services: 

The following services are currently provided only on appointment basis. Applicants requiring these services on urgency basis may send email to cons3.milan@mea.gov.in detailing their service, passport number and nature of urgency. Upon receipt of conformation email from the Consulate the applicant can visit with duly complete application within the alloted time slots.

C. Life Certificate: 

Life Certificate (Click here for application formApplication form should be in duplicate, with photographs affixed on both the copies. In addition, Miscellaneous Application form (Click here for application form ), duly completed with one photograph pasted on it, and signed by the applicant (in the presence of the Consular Officer), clear copies of (1) the relevant pages of the valid Indian Passport/OCI card (in duplicate), (2)Permesso d’Soggiorno, (3) Residence Proof (Carta d’Identita, any Utility bill, etc.) are also required.

D. Power of attorney: 

Application form should be submitted in Miscellaneous Application form (Click here for application form), duly completed with one photograph pasted on it, and signed by the applicant. Please also submit clear copies of (1) the relevant pages of the valid Indian Passport (two copies), (2)Permesso d’Soggiorno, (3) Residence Proof (Carta d’Identita, any Utility bill, etc.) are also required, and (4) Power of Attorney, in duplicate, with a passport size photograph pasted on the original as well as on the copy, (5) one clear copy of the Indian Passport or OCI card should be attached with the original Power of Attorney.

E. Affidavit for applying for passport of minor applicant in India

As per new rules, if either of the parent is in India, there is no need for an Affidavit to get attested from Indian Missions/ Posts abroad. The parent, who submits application for passport in respect of minor children, can give an Affidavit in India itself. If however, the parent, who is in a foreign country, wants to give an Affidavit, giving his/her consent, the following are required:

Application form should be submitted in Miscellaneous Application form (Click here for application form), duly completed with one photograph pasted on it, and signed by the applicant.

Please also submit:

1. Affidavit(Click here for application form) if one of the parents is in the jurisdiction of this Consulate,

OR

Affidavit (Click here for application form) if both the parents are in the jurisdiction of this Consulate and they want to authorize their relative or friend to apply for passport in respect of their minor children,

2. Clear copies of (1) the relevant pages of the valid Indian Passport (two copies) of the applicant(s), (2)Permesso d’Soggiorno, (3) Residence Proof (Carta d’Identita, any Utility bill, etc.), (4) copies of the relevant pages of the passport(s) of the children, if applying for renewal or copy of the birth certificate of the children, if applying, afresh.
