A. Steps to follow:

Read the following document carefully.
Fill online passport application with correct information and required changes, if any(documentary proof to be shared for changes).
Take printout of the application form, it comprises four pages.
Preview of your New Passport is available on the first page of your application.
Paste two recent photographs and sign on the application in the allotted boxes.
Arrange all mandatory documents in the same order as given in the table against each Passport service. All documents must be self-attested/ signed by both parents in case of Minors.
Prepare Demand draft / Assegno Circolare from Bank or Post Office for the correct fee in favor of ‘Consulate General of India’. Incorrect fee shall cases shall not be entertained.
Prepare for Postal delivery and return of ready passport either by customized DHL link or by Posta Italiane (Busta Pre-affrancata Posta Raccomandata, A5 size)
Send the prepared file by Post.
Trace the application status at http://www.cgimilan.gov.in/.

B. General instructions:

Application form can be filled through this URL https://embassy.passportindia.gov.in/.
All applications for a passport service, whether for a new passport, re-issue of passport upon expiry or loss/theft/damage, change in the entries in the existing passport (including complete/partial name change, splitting of Name-Surname, change of name after marriage, change in appearance, change in date of birth, change in place of birth, change in gender, change in signature or exhaustion of pages in the existing passport booklet) will be made through this link only.
Passport can be applied for reissue from one year before its expiry or for change in existing particulars.
Please note that birth registration of a child born to Indian parents should be done at the Consulate within one year of the birth, otherwise there will be delay in issuing a passport to the child.
Applicants must provide their Email Ids and contact number in the application form for follow up in case additional documents are required.
Applicants may be called upon at the Consulate with additional documents for verification.
Applications received without proper/matching signature and photo corresponding to the required standards shall not be entertained. Applications received with incomplete supporting documents are liable to be rejected; fee won't be refunded in such cases.

C. Who can apply:

Indian Nationals living in the areas of Lombardy, Piedmont, Aosta Valley, Trentino-Alto Adige, Friuli Venezia Giulia, Veneto, Liguria and Emilia-Romagna can apply for Passport services by POST at the Consulate General of India in Milan.

D. How to apply:

Applications can be sent only by Postal modes. Prepaid self-addressed Envelope for return of Ready Passport must be provided with the postal application. Postal modes can be any of the following:

E. Fee payment:

Applicable Fee has been detailed against the services. Fees must be submitted only in form of Demand Draft (Assegno Circolare) from Bank or Post Office in favor of Consulate General of India.

F. Guidelines on filling the online application form:

Fill the online application form at https://embassy.passportindia.gov.in/, and take printout of the application form. Please fill the application form with utmost care and provide correct information. The application form consists of four pages.
Passport application should be filled with the same details as the existing passport, except in case of desired changes. Any corrections in the existing passport must be supported with documents.
Sign the application form at earmarked space (within the allotted box) on the first page and on the space provided on the last page. All signatures on the application should be by a ball point pen of blue or black ink and have to be dark. Those who cannot affix their signatures / illiterate should put their thumb impression in the place of signature.
1. Left hand thumb impression in the case of male applicant, and
2. Right hand thumb impression in the case of female applicant.
Signature should match with the signature in the existing passport. If the applicant wants to change the signature, he/she may do so.
1. In that case, new signature should be appended in the box provided on the first page of the application form.
2. And, new as well as old signature should be appended at the last page ( New signature in the given box on last page and Old signature at the blank space below the line)
Two recent color photographs. Click here for photo specifications. Two photographs (not older than 3 months) must be pasted on the application
1. One photograph to be pasted at the space provided on the first page of the application form.
2. Second photograph to be pasted at the blank space available on fourth/last page of the application form after the declaration part.

G. Change in Appearance:

If there is a major change in appearance of the applicant in comparison to photo on the available passport, (this can be due to Age or Turban), it is mandatory to provide Affidavit for Change in Appearance Click here for Affidavit.

Passport can be applied by Postal modes for the following cases:

Application for Adults
Application for Minors
Application for Newly born applicant
Application for Lost/Damaged Passport
Application for splitting of name/surname
Application for change of name
Application for addition or deletion of Spouse name after Marriage/Divorce.

I. Passports Service and required documents:

Sl no	Service Category	Documents must be arranged in given order/sequence.
1)	Reissue of passport in respect of Adults	1. Duly filled and signed Passport Application form (to be filled online at https://embassy.passportindia.gov.in/) 2. Two Photographs (5cms x 5 cms). Click here for photo specifications, pasted on the application. 3. Original Passport and clear copies of the only relevant pages (first page, last page and endorsement pages) of the passport of the applicant (copies to be self-attested). 4. Legalized copy of valid Permesso Di Soggiorno of the applicant. It is Mandatory to provide copy Legalized by Comune (copia conforme). If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should be attached. 5. Clear copy of Address proof of the applicant (self-attested) (Carta D’Identita or any other proof like any Utility bill-electricity/water/fuel bill or rent agreement, Ospity along with translation in English). 6. Declaration that the applicant has not acquired citizenship/ passport/travel document of Italy or any other country and no criminal case has been filed in India or criminal proceedings are pending before a court in India (Click here for declaration) 7. Affidavit for change of appearance if applicable (Click here for Affidavit). 8. Fee details: For Reissue of Normal passport (36 pages): a) 62 Euros : for without change in appearance b) 87 Euros : for applications with Affidavit for change in appearance, when passport is valid for more than one year. Fee for Jumbo passport (60 pages): a) 77 Euros : for without change in appearance b) 102 Euros : for applications with Affidavit for change in appearance, when passport is valid for more than one year.
2)	Reissue of passport in respect of Minors : children below the age of 18 years	1. Duly filled and signed Passport Application form (to be filled online at https://embassy.passportindia.gov.in/). 2. Two Photographs (5cms x 5 cms) Click here for photo specifications pasted on the application. 3. Original Passport of the minor applicant and clear copies of the relevant pages (attested by parents). Copies must be attested by both the parents, signature of parents should match their passport. 4. Copy of valid Permesso di Soggiorno of the Minor applicant if available. (Signed by both the parents). If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should be attached. 5. Clear copy of Passport of both the parents with relevant pages (first, last and endorsement pages, self-attested by parents). 6. Clear copy of Permesso di Soggiorno of both the parents. Copy of Soggiorno of either parent must be Legalized by Comune (copia conforme). If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should also be submitted (self-attested by parents). 7. Clear copy of the Address proof (Carta D’Identita or any other proof like any Utility bill-electricity/water/fuel bill or rent agreement, Ospity and its translation in English) of the parents (self-attested by parents). 8. Declaration from both parents for applying Passport of the Minor applicant (Click here for Annexure D). 9. Fee details: a) 42 Euors( age less than 15 years) b) 62 Euors ( age more than 15 years)
3)	Fresh Passport for Minors/ Passport for Newly borns	Application for new born comprises of two stages: a) Birth registration with the Consulate, and b) Passport application Birth registration application needs to be filled online at https://indiancitizenshiponline.nic.in/ under category ‘Registration of birth of a minor child at an Indian Consulate Under section 4(1) of the Citizenship Act, 1955’. The printout of the birth registration form needs to be submitted after signature by both the parents twice in the space provided. Note: Please make sure that both parents sign twice on the available space, the signatures should be same as per their Passport. Documents must be arranged in the following order: 1. Duly filled and signed Passport Application form (to be filled online at https://embassy.passportindia.gov.in/). 2. Two Photographs (5cms x 5 cms) Click here for photo specifications pasted on the application. 3. Printout of Duly filled child's birth registration form (to be filled online at https://indiancitizenshiponline.nic.in/). Birth registration form without proper and matching signatures of both parents cannot be accepted and Passport would not be granted in such cases. 4. Original and clear photocopy of Birth certificate issued from Comune(if born in Italy) either Multilingual or with English translation having the name of the child, date of birth, place of birth, gender of the new born and names of both the parents (self-attested by parents). If the child is born outside Italy, attested copy of birth certificate from the Embassy / Consulate of the Country to be submitted along with apostillation by Prefettura. 5. Clear copy of valid Permesso Di Soggiorno of the child, if issued already (self-attested by parents). 6. Clear copy of Passport of both the parents with relevant pages (first, last and endorsement pages, self-attested by parents). 7. Clear copy of Permesso di Soggiorno of both the parents. Copy of Soggiorno of at least one parent must be Legalized by Comune (Copia Conforme). If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should also be submitted (self-attested by parents). 8. Clear copy of the Address proof (Carta D’Identita or any other proof like any Utility bill-electricity/water/fuel bill or rent agreement, Ospity and its translation in English) of the parents (self-attested by parents). 9. Clear copy of the Marriage Certificate (self-attested) of the parents. 10. Declaration from both parents for applying Passport of the new born (Click here for Annexure D) 11. If the birth of the child is not registered within one year of the birth, the reason for delay in registering the birth, in writing, duly signed by both the parents, is required. 12. Fee details: 63 Euros(including Passport and Birth registration)
4)	In lieu of Lost Passport / Damaged Passport	1. Duly filled and signed Passport Application form (to be filled online at https://embassy.passportindia.gov.in/) 2. Two Photographs (5cms x 5 cms). Click here for photo specifications pasted on the application. 3. Clear copies of the relevant pages as available (first, last, Visa and endorsement pages) of the passport of the applicant (self-attested). 4. FIR for the Lost passport with number of the lost passport mentioned in the report and English translation. 5. Legalized copy of valid Permesso Di Soggiorno of the applicant. It is Mandatory to get the copy Legalized by Comune (copia conforme). If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should be attached. 6. Clear copy of Address proof of the applicant (self-attested) (Carta D’Identita or any other proof like any Utility bill-electricity/water/fuel bill or rent agreement, Ospity along with translation in English). 7. Duly filled Declaration as per ‘Annexure F’ for applying reissue of passport in lieu of Lost passport (Click here for Annexure F) 8. Two copies of PP form (personal particulars) form filled in completely and with latest photograph(Click here for PP form) 9. Affidavit for the change in appearance if applicable. (Click here for Affidavit) 10. Fee details: Fee for normal passport (36 pages): a) 112 Euros : for without change in appearance b) 137 Euros : for applications with Affidavit for change in appearance, when passport is valid for more than one year. Fee for Jumbo passport (60 pages): a) 132 Euros : for without change in appearance b) 157 Euros : for applications with Affidavit for change in appearance, when passport is valid for more than one year.
5)	Reissue for split name: (Nome Cognome)	1. Duly filled and signed Passport Application form (to be filled online at https://embassy.passportindia.gov.in/) 2. Two Photographs (5cms x 5 cms) Click here for photo specifications pasted on the application. 3. Original Passport and clear copies of the relevant pages (first, last and endorsement pages) of the passport of the applicant (self-attested). 4. Legalized copy of valid Permesso Di Soggiorno of the applicant. a) If the applicant has Permesso Di Soggiorno; it is Mandatory to get the copy Legalized by Comune (copia conforme). If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should be attached. b) If the applicant does not have Permesso Di Soggiorno; the following must be submitted: i. Clear copies of the (a) Passport, (b) Permesso D’Soggiorno and (c) Carta D’Identita of the relative on whose family Visa the applicant arrived. ii. There should be documentary proof to substantiate the relationship, such as Marriage Certificate, Birth Certificate, etc. 5. Affidavit for change of appearance if applicable ( click here for Affidavit) 6. Fee details: For Reissue of Normal passport (36 pages): a) 62 Euros : for without change in appearance b) 87 Euros : for applications with Affidavit for change in appearance, when passport is valid for more than one year. Fee for Jumbo passport (60 pages): c) 77 Euros : for without change in appearance d) 102 Euros : for applications with Affidavit for change in appearance, when passport is valid for more than one year.
6)	Reissue of passport for Change in name	Documents remain same as per respective categories detailed above, in addition the following supporting documents must be provided: 1. Sworn affidavit for the change of name ( click here for specimen) 2. Newspaper advertisement for change of name in two daily newspapers from India in any ‘national daily’ and one in Italy. Original newspaper to be submitted, the advertisement must not be older than one year. 3. Two documents issued by authorities in India with desired Name. 4. Two documents issued by Local authorities in Italy with desired name in case of Permesso Di Soggiorno is Illimitata. 5. Fee details: For normal passport (36 pages): a. 87 Euros : for without change in appearance b. 112 Euros : for applications with Affidavit for change in appearance, when passport is valid for more than one year. Fee for Jumbo passport (60 pages): a. 102 Euros : for without change in appearance. b. 127 Euros : for applications with Affidavit for change in appearance, when passport is valid for more than one year.
7)	Reissue of passport with addition/deletion of spouse’s name	Documents remain same as per respective categories detailed above, in addition the following supporting documents must be provided: 1. In case to add spouse’s name on the passport: Clear copy of the Marriage Certificate (self- attested). In case the marriage was solemnized in Comune in Italy, the marriage certificate should be apostilled from Prefettura and the translation in English is mandatory. In case the marriage was solemnized in any other country, the marriage certificate, in English, should be attested from the concerned Indian High Commission /Embassy/Consulate. 2. In case of deletion of spouse’s name on the passport: Clear copy of “Divorce Decree” from the court pertaining to divorce apostilled by the Ministry of External Affairs India. If “order” issued in Italy same should be apostilled from Prefettura and translation in English is mandatory. 3. Fee details: For normal passport (36 pages): a. 62 Euros : for without change in appearance b. 87 Euros : for applications with Affidavit for change in appearance, when passport is valid for more than one year. Fee for Jumbo passport (60 pages): a. 77 Euros : for without change in appearance. b. 102 Euros : for applications with Affidavit for change in appearance, when passport is valid for more than one year.
