The passport can be applied for reissue from one year before its expiry or for change in the entries in the existing passport including complete name change, change of name after marriage, change in appearance, change in date of birth, change in place of birth, change in sex or change in signature or exhaustion of pages.

Collection of serviced passports for various Miscellaneous services for which applications are submitted after 1200 hours: 1500 hours to 1600 hours

All applicants, irrespective of minor children (except new born children) have to submit Application for passports, in person.

Passport, on being re-issued, has to be collected by applicants themselves except in the following cases: Husband/wife can collect the passports of his/her spouse with documentary proof of their relationship. Passports of minor children (below the age of 18 years at the time of collection) can be collected by either of their parents with proof of parentage.

All the columns including telephone no. in the application form should be filled properly without any ambiguity and no column should be left blank. The residential address in India and Italy must be entered against permanent address(address to be printed on the Passport) and any other address respectively. The applicant should carefully check the particulars entered in the application form. The incomplete forms or having wrong particulars will NOT be processed further.

All signatures in the application should be by a ball point pen of blue or black ink and have to be dark. Those who cannot affix their signatures like infants, minor children or illiterate should put their thumb impression (left hand thumb impression in the case of male and right hand thumb impression in the case of female) in the place of signature.

Please bring original as well as clear copies of all documents. Copies of all the documents should be self-attested by the applicants. In case of minor children, copies of all the documents should be self-attested by the parents. Original documents will be returned immediately after verification. Incomplete applications will not be accepted.

PASSPORTS

Please submit the application in the following order:

I. REQUIREMENTS FOR APPLYING FOR PASSPORTS IN CASE OF ADULTS

1. APPLY ON-LINE at http://passport.gov.in/nri/OnlineRegistration.jsp?pocode and take a print out of the Passport application form. Please paste two recent colour photographs of the size of 2 inch x 2 inch with light back ground, one on the first page where space is provided and the second one on the blank space on the third page of the application. The photograph should have a frontal view, showing both the ears. The eyes must be open and no hair should obscure the face. Prescription glasses if worn should be clear and thin framed and should not have flash reflection or obscure the eyes. Head coverings, hair, head-dress or facial ornaments should not obscure the face. Head should face the camera directly and should not tilt or turned (portrait style). Head covering is not permitted except for religious reasons, but the facial features from bottom of chin to top of forehead and both edges of the face, with both ears must be clearly visible.

2. All columns in the Passport application form should be filled in neatly and in CAPITAL LETTERS. No column should be left blank.

3. Please append your signature at two places, one in the column provided in the first page of the application and second one on the second page, below Sl.No.26 (Under Declaration). Signature should match with that in their existing passport.

4. If the applicant wants to change the signature, he/she may do so. In that case, new signature should be appended in the column provided in the first page of the application form. New as well as old signature should be appended below Sl.No.26 (Under Declaration).

5. If the applicant wants to change any personal particulars (name, surname, date of birth, place of birth, spouse name, correction of spelling mistakes, change in permanent address, etc.), it can be done provided there is documentary proof for the same. In case the applicant wants to change name or surname, an advertisement should be published in two newspapers in India (one in regional language newspaper and the second one in English newspaper) and in newspaper in Italy. In such case, application can be submitted after thirty days of publication of the advertisement in the last published newspaper. Original newspaper should be submitted. An Affidavit (Click here for the Affidavit*2) should also be submitted.

6. Clear copies of the relevant pages of the passport (self-attested)

7. Clear copy of the valid Permesso Di Soggiorno(self-attested). If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should also be submitted.

8. Clear copy of the Address proof (Carta D’Identita or any other proof like any Utility bill-electricity/water/fuel bill or rent agreement, Ospity along with translation in English) (self-attested).

9. In case the applicant wants to change her surname consequent on her marriage and replace her existing surname with her husband’s name, an Affidavit((Click here for the Affidavit*3) may be submitted along with a Joint Photograph (colour) of the size of 8 cm x 6 cm. The Joint Affidavit should be signed by both the husband and wife at the Consulate in the presence of the Consular Officer.

10. IN CASE THE APPLICANT IS ON FAMILY VISA AND WANTS TO GET PASSPORT WITH SPLIT NAME (NOME AND COGNOME) : Clear copies of the (a) passports, (b) Permesso D’Soggiorno and (c) Carta D’Identita of the relative on whose family visa the applicant arrived, and there should be documentary proof to substantiate the relationship, such as Marriage Certificate, Birth Certificate, etc. In addition, please submit Receipt for applying for Permesso Di Soggiorno and the communication from Comune/Qustura intimating the date for biometrics (finger print, etc.).

11. Clear copy of the Marriage Certificate (SELF-ATTESTED), if married and if spouse name is not endorsed on the passport. In case the marriage was solemnized in Comune in Italy, the marriage certificate should be apostilled from Prefutura and translation in English is mandatory. In case the marriage was solemnized in any other country, the marriage certificate, in English, should be attested from the concerned Indian High Commission/Embassy/Consulate.

12. In the case of APPLYING FOR DUPLICATE PASSPORT IN LIEU OF LOST OR DAMAGED PASSPORT, please submit original as well as copy of the FIR (Police Report) along with its English translation(SELF ATTESTED) regarding loss of passport, clearly mentioning the number of the lost passport. In such case of loss of passport, Annexure ‘L’ ( Click here for Annexure ‘L’*4), duly completed and signed by the applicant needs to be submitted. In addition, Personal Particulars Form( Click here for Personal Particulars Form*5), in triplicate, with three photographs pasted on each copy, duly filled in and signed should also be submitted.

REQUIREMENTS FOR APPLYING FOR PASSPORTS IN RESPECT OF CHILDREN BELOW THE AGE OF 18 YEARS

Both the parents should append their signatures (signature should match with that in their passports) on the third page of the application form (just above, where it is written signature of parents... mother... father). If one of the parents is away from the country of domicile (in this case, away from Italy), the other parent, who submits the application should submit an Affidavit ((Click here for the Affidavit*6).

REQUIREMENTS FOR APPLYING FOR PASSPORTS FOR NEW BORN CHILDREN

1. Register the Birth, on-line at : https://indiancitizenshiponline.nic.in/, within one year of the birth of the child. Upload the photograph of the child, signatures of both the parents, copies of the passports of both the parents, marriage certificate of the parents and birth certificate of the child (in English). Please click Registration of birth of a minor child at an Indian consulate under section 4(1) of the citizenship act, 1955

2. If the birth of the child is not registered within one year of the birth, the reason for delay in registering the birth, in writing, duly signed by both the parents, is required. If the birth of the child is not registered within one year of the birth, the process may take longer time.

3. Take a print out of the completed form and submit along with the passport application form.

4. Parents should append their signatures (signature should match with that in their Passports) at two places on the second page of the Birth Registration application (where it is written Signature of parents). Both the parents should append their signatures (signature should match with that in their passports) on the third page of the application form (just above, where it is written signature of parents... mother... father). If one of the parents is away from the country of domicile (in this case, away from Italy), the other parent, who submits the application should submit an Affidavit ((Click here for the Affidavit*6).

6. Please note that since a new born baby cannot sign, his/her thumb impression (left hand in case of male child and right hand in case of female child) should be appended on the first page of the passport application form (space provided).

7. Both parents should append their signatures (signature should match with that in their passports) on Page No.2 of the application (Under Sl. No.26..DECLARATION) and also on the third page of the application form. Both the parents should append their signatures (signature should match with that in their passports) on the third page of the application form (just above, where it is written signature of parents... mother... father). If one of the parents is away from the country of domicile (in this case, away from Italy), the other parent, who submits the application should submit an Affidavit ((Click here for the Affidavit*6).

8. Copy of the birth certificate issued by Comune along with its English translation (self attested by parents). The birth certificate should have the name of the child, date of birth, place of birth, gender and names of both the parents.

9. Clear copy of valid Permesso Di Soggiorno of the child, if issued(self-attested by the parents)

10. Clear copies of all the relevant pages of the passports of both the parents(self-attested by parents)

11. Clear copies of Permesso D’Soggiorno of both the parents (self-attested by parents). If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should also be submitted (self-attested by parents).

12. Clear copy of the Address proof(self-attested)( Carta D’Identita or any other proof like any Utility bill-electricity/water/fuel bill or rent agreement, Ospity and its translation in English) of the parents.

13. Copy of the Marriage Certificate(self-attested) of the parents if their names are not endorsed on each other’s passport.

HOW TO TAKE APPOINTMENT FOR SUBMISSION OF APPLICATION FOR PASSPORT

Once you click on the Button “for Submission of Applications”, it will take you to a screen as under:


Click on the Service you require, i.e., “Passport Service” and it will take you to another screen, as under :

Please enter your Application Number, which is the WEBFILE NUMBER (ITAMP……) created after completion of your application form for Passport. After entering the Application ID, click on the SEARCH button, it will take you to the next screen, as under;

Click on the button “Click here for registration”. Once you click on the button, it will take you to another screen, as under :

Please enter your Passport Number and your name followed by surname. Thereafter, please enter the Verification Code and click on the button “Register”. The next screen will appear, as under :

You will be able to take appointment from the afternoon of the next working day of the Consulate from the date of registration.

When you click on the BOOK YOUR APPOINTMENT ONLINE, next day afternoon, for taking appointment, the screen will appear as under :

Once you select Passport Service, the screen will appear as under :

Please enter the Application Number (ITAMP……) and click on “Search”, the screen will appear as under :

Please click on the button “CLICK HERE for appointment”. It will take you to another screen as under :

Please enter the Verification Code and click on “CHECK AVAILABILITY”. It will take you to another screen where you can see the calendar, as under:

Available dates are shown in GREEN colour. If no dates are available, the dates will be displayed in RED colour. If no date is available in the particular month, you can click on the button “Next Month shown on the right side of the calendar. You can choose the date as per your convenience and click on the date.

Once you click on the chosen DATE, please click on the “OK” button and it will take you to the next screen as under :

As can be seen, the date you selected turns into BLUE. Please click on “OK” button and your appointment is booked for the day you selected and the confirmation will be displayed as under :
