About Us Postal applications for Consular Services

Consulate General of India, Milan will start receiving applications for Passport/OCI/Miscellaneous Consular Services on ‘Scheduled Appointment Basis’ only with effect from 8th November, 2021 (Monday).
 
Receipt of Postal Applications through DHL or any other courier services will be stopped with effect from 15th October, 2021. All applicants are therefore advised NOT repeat NOT to send the applications through any Courier Services after 15th October, 2021(Friday).
 
All applicants are further requested to please bring the following:-
 
a) Duly filled in application forms along with copies of all necessary supporting documents;
b) Online Payment receipt made at least 3 (three) working days in advance, prior to the date of appointment and
c) Copy of valid Green Pass.
 
Considering Covid protocol and restrictions, please note that entry to the Consulate will be strictly restricted to Green Pass holders only.

B. Steps to follow:

  1. Read the following instructions carefully.
  2. Fill the Miscellaneous Application form. Click here to download the application form.
  3. Paste a recent photographs and sign on the application in the allotted boxes on first and last page. Mention the service category clearly on the application form.
  4. Arrange all mandatory documents in the same order as listed below with each service. All documents must be self-attested by applicant or by both parents in case of Minor applicant.
  5. Submit correct fee to Bank account of the Consulate. Take printouts of the transaction summary to be attached with your application as payment proof.
  6. Book appointment online.

C. Who can apply:

Indian Nationals living in the areas of Lombardy, Piedmont, Aosta Valley, Trentino-Alto Adige, Friuli Venezia Giulia, Veneto, Liguria and Emilia-Romagna can apply for Miscellaneous Consular services at the Consulate General of India in Milan.

D. Fee payment:

  • Additional fee of Euro One shall be levied in respect of various Passport, Consular, OCI and Visa services with effect from 1st June 2021 towards mandatory bank charges. Fee can be submitted through Bank transfers. Click here for detailed information. 

1. Same Person Certificate

This certificate is issued as per Passport particulars of the applicant to identify the person in case of minor mistake on other Indian documents. Following documents are required to issue the certificate;
 
(i) Duly complete and signed Miscellaneous application form.
(ii) Original Indian Passport and clear copy.
(iii) Clear copy of MEA apostilled birth certificate/marraige certificate/PCC etc having minor change in personal particulars.
(iv) Legalised copy (copia legalizzata) of a valid or expired Permesso Di Soggiorno along with copy of ricevuta if applied for reissue of Permesso Di Soggiorno.
(v) Copy of Residence Proof Carta d’Identita, ospitalità or Utility bill, etc. (Self attested).
Fees:  34 Euros + 2 Euros ICWF +1* = 37 Euros.

2. Identity Certificate:

Identity Certificate is issued as per the Passport particulars of the applicant. Following documents are required for issuing Identity Certificate.
 
(i) Duly complete and signed Miscellaneous application form.
(ii) Original Indian Passport and clear copy.
(iii) Legalised copy (copia legalizzata) of a valid or expired Permesso Di Soggiorno along with copy of ricevuta if applied for reissue of Permesso Di Soggiorno.
(iv) Copy of Residence Proof Carta d’Identita, ospitalità or Utility bill, etc. (Self attested)
Fees: 34 Euros + 2 Euros ICWF +1*= 37 Euros

3. Family Certificate.

Family certificate is issued to only Indian Passport holder whose close family are resident in Italy or in India. In case the family is in India affidavit from parents duly apostilled from Patiala house is required.
The following documents are required to issue this certificate:
 
(i) Duly complete and signed Miscellaneous application form.
(ii) Copy of the valid Indian Passport of self and family. Birth certificate/Aadhar card of family member who don’t have Passport.
(iii) Legalised copy (copia legalizzata) of a valid or expired Permesso Di Soggiorno along with copy of ricevuta if applied for reissue of Permesso Di Soggiorno.
(iv) Copy of marriage certificate in case of spouse name is required on Family certificate.
(iv) Copy of Residence Proof Carta d’Identita, ospitalità or Utility bill, etc. (Self attested)
Fees:  34 Euros + 2 Euros ICWF +1*= 37 Euros

4. Registration of Death, No Objection Certificate for local cremation and for transfers of Mortal remains to India

The Consulate can issue No Objection Certificate for cremation, burial, and transporting dead body to India if the deceased was holder of an Indian Passport and was residing under the jurisdiction of the Consulate.
Following documents are required to issue such certificates:
 
(i) Original and copy of Indian Passport of the deceased.
(ii) Copy of resident permit of the deceased.
(iii) In case of an accident/murder/suicide No objection from Police/ Court for cremation/burial or transportation of the mortal remains to India.
(iv) Medical report stating cause of death with English translation.
(v) Death Certificate issued by Comune (Prulingue) or with English translation in case of Italian.
(vi) Letter from the Funeral/Transport Agency, taking responsibility for completing all the required formalities.
(vii) In case transporting of dead body to India Embalming Certificate clearly stating that it is not contagious/dangerous in any way to the health of passengers in the flight.
(viii) In case, close relatives of the deceased are in India, Consent letter or authority letter or affidavit signed by the close relative(spouse/son/daughter/parents) of the deceased. It should be duly attested by Village Officer/ Tehsildar/Panchayat President, with date and seal giving the name and designation of the officer who attests the document and his/her office seal. Attestation by Notary Public will be accepted only if the same is attested by the respective Court. Please enclose identity proof (Passport, Aadhar Card, etc.) of both the persons (person who is giving consent and to whom it is given).
(ix) In case the close relative is in Italy, a self declaration with supporting documents (Passport copy and identity proof) to be submitted to the Consulate.
(x) If a person wants to accompany a dead body to India an authorisation letter is issued from the Consulate. Copy of Passport and identity proof of individual is required to get the authority letter.
 
Fees:  17 Euros + 2 Euros ICWF +1*= 20 Euros

5. No Objection Certificate for transfer of Mortal remains(ashes) of foreign nationals of Indian origin

(i) Duly complete and signed Miscellaneous application form.
(ii) In case, close relatives of the deceased are in India, Consent letter or authority letter or affidavit signed by the close relative(spouse/son/daughter/parents) of the deceased. It should be duly attested by Village Officer/ Tehsildar/Panchayat President, with date and seal giving the name and designation of the officer who attests the document and his/her office seal. Attestation by Notary Public will be accepted only if the same is attested by the respective Court. Please enclose identity proof (Passport, Aadhar Card, etc.) of both the persons (person who is giving consent and to whom it is given).
(iii) In case the close relative is in Italy, a self declaration with supporting documents (Passport copy and identity proof) to be submitted to the Consulate.
(iv) Certificate for cremation, Cause of Death Certificate(ISTAT) with english translation and Death Certificate(Prulingue).
 
Fees:  34 Euros + 2 Euros ICWF + 1* = 37 Euros
 

6. Certificate of Birth

Certificate of Birth is issued if the applicant is born under the jurisdiction of this Consulate. In other cases, a general certificate stating the date of birth and place of birth as per passport shall be issued. Following documents are required for issuing Birth Certificate from the Consulate.
 
(i) Duly complete and signed Miscellaneous application form.
(ii) Original Indian Passport and clear copy.
(iii) Legalised copy (copia legalizzata) of a valid or expired Permesso Di Soggiorno along with copy of ricevuta if applied for reissue of Permesso Di Soggiorno,
(iv) Copy of Residence Proof Carta d’Identita, ospitalità or Utility bill, etc. (Self attested)
Fees: 34 Euros + 2 Euros ICWF +1*= 37 Euros

7. Unmarried Certificate/ Bachelorhood Certificate for solemnisation of marriage at Comune in Italy.

The Consulate can issue Bachelorhood Certificate (Unmarried Certificate), if only the same is issued by the competent authority in India (Assistant Commissioner of Police, Deputy Commissioner of Police or above, an officer of not below the rank of First Class Judicial Magistrate, etc.,) and has been duly apostilled/attested from Ministry of External Affairs, New Delhi.
In case such a certificate is not being issued from the concered authority the applicant can obtain a declaratory order from the competent Court having jurisdiction in the area of residential address of the applicant, this Declaratory Court-order has first to be counter-attested by the concerned State Home Department and then apostilled by MEA.
The following documents are required to issue this certificate;
(i) Original Bachelorhood Certificate/ unmarried certificate / marriage ability certificate or Declatory Order issued by the competent authority in India (Assistant Commissioner of Police, Deputy Commissioner of Police or any officer not below the rank of First Class Judicial Magistrate). The certificate should be duly apostilled by Ministry of External Affairs, India. 
(ii) Miscellaneous Application form duly completed with one photograph pasted on it and signed by the applicant.
(iii) Original Indian Passport.
(iv) Legalised copy (copia legalizzata) of a valid or expired Permesso Di Soggiorno along with copy of ricevuta if applied for reissue of Permesso Di Soggiorno.
 (vi) Copy of Residence Proof Carta d’Identita, Ospity or Utility bill, etc. (Self attested)
Fees: 34 Euros + 2 Euros ICWF +1*= 37 Euros

8. True copy attestation of Indian Passport

(i) Duly complete and signed Miscellaneous application form.
(ii) Original Indian Passport with clear copy.
(iii) Legalised copy (copia legalizzata) of a valid or expired Permesso Di Soggiorno along with copy of ricevuta if applied for reissue of Permesso Di Soggiorno,
(iv) Copy of Residence Proof Carta d’Identita, ospitalità or Utility bill, etc. (Self attested)
Fees: 9 Euros + 2 Euros ICWF +1*= 12 Euros

9. Extract of Marriage certifcate

(i) Duly complete and signed Miscellaneous application form.
(ii) Original Indian Passport of the applicant.
(iii) Clear copy of Indian passport of spouse.
(iv) Clear copy of MEA apostilled Marriage certificate.
(iv) Legalised copy (copia legalizzata) of a valid or expired Permesso Di Soggiorno along with copy of ricevuta if applied for reissue of Permesso Di Soggiorno.
(v) Copy of Residence Proof Carta d’Identita, ospitalità or Utility bill, etc. (Self attested).
Fees:  34 Euros + 2 Euros ICWF +1*= 37 Euros.

10.  'Certificate/Receipt' for reissue of International Driving Permit(IDP) from India

The Government of India has introduced the new Miscellaneous Consular Service to facilitate the re-issuance of International Driving Permit (IDP) to those Indian nationals, who were originally issued IDP by the concerned authorities in India. The Consulate will only issue a prescribed ‘Certificate/Receipt’ against the Fee of Euro 14.00 and NOT an IDP to the applicant. The applicant would need to upload all the relevant documents along with the ‘Certificate/Receipt’ issued by the Consulate in the portal www.parivahan.gov.in and pay the requisite IDP fees online on the same portal.  On receipt of the application through the portal, the licensing authority under Ministry of Road, Transport and Highways (MoRTH), upon verification of the documents shall courier the IDP directly to the residential address of the applicant.

It may be noted that the role of the Consulate in this consular service is limited to the extent of facilitating submission of IDP renewal application in the portal www.parivahan.gov.in only.  Any further correspondence pertaining to the subject, including status of the application, would need to be verified directly from the concerned authority (MoRTH) in India.

Documents required for the above certificate/receipt:
(i) Miscellaneous Application form duly completed with one photograph pasted on it and signed by the applicant. 
(ii) Original Indian Passport with clear copy.
(iii) Original valid Indian License and International Driving Permit with clear copies.
(iv) Legalised copy (copia legalizzata) of a valid or expired Permesso Di Soggiorno along with copy of ricevuta if applied for reissue of Permesso Di Soggiorno.
(v) Copy of Residence Proof Carta d’Identita, ospitalità or Utility bill, etc. (Self attested)
Fees: 9 Euros + 2 Euros ICWF +1* = 12 Euros.

11. Attestation of Commercial Documents:

(i) A letter from the company, in English, authorizing the bearer of the documents to get them attested from the Indian Consulate mentioning reason for attestation.
(ii) Legalised copy of valid Passport or Carta d’Identità from Comune/Prefettura.
(iii) Commercial documents, in duplicate (must be Apostilled from Prefettura).
Fees: 42 Euros + 2 Euros ICWF +1*= 45 Euros
Note: Attestation of commercial documents can be done only if the documents are apostilled from Prefettura.

12. Attestation of translation of Indian Documents

In compliance with the instructions of Government of India, all the applicants, who seek Consular Services for attestation of 'Translation of the Document' in Italian are henceforth requested to submit the legalised/notarised copy of the translation from the 'Traduzione Giurata'; along with other required documents as detailed under:
The following documents are required for attestation of translated documents.
(i) Miscellaneous application form duly completed and signed by the applicant. 
(ii) Original as well as copy of the documents required for attestation, in duplicate.
(iii) Original Indian Passport.
(iv) Legalised copy (copia legalizzata) of a valid or expired Permesso Di Soggiorno along with copy of ricevuta if applied for reissue of Permesso Di Soggiorno,
(v)Copy of Residence Proof Carta d’Identita, any Utility bill, ospitalità etc. (Self attested)
Fees:  17 Euros + 2 Euros ICWF +1*= 20 Euros
Note: If the document is issued from India, the same should be apostilled from Ministry of External Affairs, India. If it is issued by the Italian Government or private organization, it should first be apostilled from Prefettura and thereafter, submitted along with translation in English.
