Consulate General of India, Milan will start receiving applications for Passport/OCI/Miscellaneous Consular Services on ‘Scheduled Appointment Basis’ only with effect from 8th November, 2021 (Monday).
 
Receipt of Postal Applications through DHL or any other courier services will be stopped with effect from 15th October, 2021. All applicants are therefore advised NOT repeat NOT to send the applications through any Courier Services after 15th October, 2021(Friday).
 
All applicants are further requested to please bring the following:-
 
a) Duly filled in application forms along with copies of all necessary supporting documents;
b) Online Payment receipt made at least 3 (three) working days in advance, prior to the date of appointment and
c) Copy of valid Green Pass.
 
Considering Covid protocol and restrictions, please note that entry to the Consulate will be strictly restricted to Green Pass holders only.
 

1. Power of Attorney

  1. Duly filled-in application along with the original passport of the applicant(s)and a self-attested copy of the passport(Click here for application form);
  2. Original Indian Passport alongwith self-attested copy of the passport & contact details (including local contact number) of the witnesses;
  3. Submit correct fee to Bank account of the Consulate. Take printouts of the transaction summary to be attached with your application as payment proof;
  4. Legalized copy of the Permesso Di Soggiorno;
  5. Self-attested copy of the valid Carta D’Identita of the applicant(s)Fee : 17 Euros + 2 Euros = 19 Euros

2. Declaration by Applicant’s Parent(s) or Guardian for issue of passport to minor (Annex-D)

- Duly filled-in application along with the original passport of the Parent(s) or Guardian and a self-attested copy of the passport(s) (Click here for application form and Click here for Affidavit )  ;

- Legalized copy of the ‘signature’ of the Parent(s) or Guardian either from the local Comune (Municipality) or Notaio (Notary Public);

- Submit correct fee to Bank account of the Consulate. Take printouts of the transaction summary to be attached with your application as payment proof;

- Legalized copy of the Permesso Di Soggiorno of the Parent(s) or Guardian;

- Self-attested copy of the valid Carta D’Identita of the Parent(s) or Guardian

Fee : 9 Euros + 2 Euros = 11 Euros

Kindly note that the applicant(s) must be available for a video-call for verification purposes while processing the application
