The Consulate General of India, Milan will be holding a special camp (Open House) in the Consulate (Piazza Paolo Ferrari, 8, 20121, Milan)

on 26.11.2016 (Saturday)

From 0900 hours to 1300 hours

for submission of application for PASSPORTS ONLY,

PURELY ON APPOINTMENT BASIS.

No application will be accepted without appointment.


(No other services shall be rendered)


All applicants will have to be present themselves, irrespective of their age, for submission of applications, except in the case of application in respect of new born children.


Please ensure that your application is filled in correctly and is complete in all respect and that clear copies all documents are attached.

Please bring original documents as well, which will be returned immediately after verification. Incomplete applications will not be accepted.


Please submit the application in the following order:


REQUIREMENTS FOR APPLYING FOR PASSPORTS IN CASE OF ADULTS

(18 YEARS OR ABOVE)


1. APPLY ON-LINE at http://passport.gov.in/nri/OnlineRegistration.jsp?pocode and take the print out of the Passport application form. Please paste two recently taken colour photographs of the size of 2 inch x 2 inch with light back ground, one on the first page where space is provided and the second one on the blank space on the third page of the application. The photograph should have a frontal view, showing both the ears. The eyes must be open and no hair should obscure the face. Prescription glasses if worn should be clear and thin framed and should not have flash reflection or obscure the eyes. Head coverings, hair, head-dress or facial ornaments should not obscure the face. Head should face the camera directly and should not tilt or turned (portrait style). Head covering is not permitted except for religious reasons, but the facial features from bottom of chin to top of forehead and both edges of the face, with both ears must be clearly shown.

All columns in the Passport application form should be filled in neatly and in CAPITAL LETTERS. No column should be left blank.


2. Clear copies of the relevant pages of the passport, Permesso Di Soggiorno and Carta D’Identita. In case Carta D’Identita has not been issued, please bring latest Ospity, in original, and copy of the same along with translation in English.

3. In case the applicant wants to change his/her signature, it can be done so. However, please append both the old as well as new signatures on page No.2 of the application form (under DECLARATION) and only the new signature on the first page of the application form.

4. In case of Loss of Passport, please submit original as well as copy of the FIR (Police Report) as well as its translation in English, clearly mentioning the number of the Passport, which is lost, Annexure ‘L’ and Personal Particulars Form (in triplicate) with three photographs.

5. Original as well as copy of documentary proof for change of personal particulars, if any,

6. In case the applicant is on family visa, arrived in Milan, the following additional documents may also please be submitted :

(i) clear copy of the visa,

(ii) Clear copies of the (a) passports, (b) Permesso D’Soggiorno and (c) Carta D’Identita of the relative on whose family visa the applicant arrived, and there should be documentary proof to substantiate the relationship.

7. Clear copy of Marriage Certificate, in case of endorsement of spouse name (In case the marriage was solemnized in Comune, please bring original certificate issued by the Comune, after getting it duly apostilled/attested from Prefuttura along with clear copy and also English Translation.


REQUIREMENTS FOR APPLYING FOR PASSPORTS IN RESPECT OF CHILDREN

BELOW THE AGE OF 18 YEARS,


1. APPLY ON-LINE at http://passport.gov.in/nri/OnlineRegistration.jsp?pocode and take the print out of the Passport application form. Please paste two recently taken colour photographs of the size of 2 inch x 2 inch with light back ground, one on the first page where space is provided and the second one on the blank space on the third page of the application. REQUIREMENTS FOR APPLYING FOR PASSPORTS FOR NEW BORN CHILDREN


Please submit the application in the following order:

Please note that since a new born baby cannot sign, his/her thumb impression (left hand in case of male child and right hand in case of female child) should be appended on the first page of the passport application form (space provided).

Both parents should append their signatures (exactly as given on their passports) on Page No.2 of the application (Under Sl. No.26..DECLARATION) and also on the third page of the application form.

2. Birth Registration, on-line, duly signed by both the parents (signature exactly as given in their Passports).Register the birth of the child ON-LINE at : http://indiancitizenshiponline.nic.in/ic_form_public.aspx within one year of the birth of the child. Upload the photograph of the child, signatures of both the parents, copies of the passports of both the parents, marriage certificate of the parents, birth certificate of the child. Thereafter, take print out of the completed form. Parents’ signature (exactly as given in their Passports) should be appended at two places on the second page of the Birth Registration application.

3. Copy of the birth certificate issued by Comune along with translation in English, if not in English,

4. Clear copies of all the relevant pages of the passports of both the parents

5. Clear copies of Permesso D’Soggiorno of both the parents as well as of the child

6. Clear copies of Carta D’Identita of both the parents as well as of the child (In case Carta D’Identita has not been issued, any other Resident Proof (Utility bill, like electricity/water/gas bills, etc) or Ospity, duly authenticated from the concerned authorities (Ospity should be the latest one with English translation).

7 Copy of the Marriage Certificate of the parents if their names are not endorsed on each other’s passport.


Entry is restricted only to those persons, who visit the Camp to submit application for passport

and not to any accompanying person.
