Separate Appointment should be taken for each applicant, irrespective of the fact they are members of the family staying together at the same address

Timings : From 0900 hours to 1230 hours

Please bring original as well as clear copies of all required documents. Original documents will be returned immediately after verification.

Applicants will have to be present in person, irrespective of their age, for submission of applications, except in the case of application in respect of new born children.

Please ensure that the application is filled in correctly and is complete in all respect and that clear copies all required documents are attached. Incomplete applications will not be accepted.

Please submit the application in the following order:

REQUIREMENTS FOR APPLYING FOR RE-ISSUE (RENEWAL) OF PASSPORTS

IN CASE OF ADULTS

1. APPLY ON-LINE at http://passport.gov.in/nri/OnlineRegistration.jsp?pocode and take a print out of the Passport application form. Please paste two recent colour photographs of the size of 2 inch x 2 inch with light back ground, one on the first page where space is provided and the second one on the blank space on the third page of the application. The photograph should have a frontal view, showing both the ears. The eyes must be open and no hair should obscure the face. Prescription glasses if worn should be clear and thin framed and should not have flash reflection or obscure the eyes. Head coverings, hair, head-dress or facial ornaments should not obscure the face. Head should face the camera directly and should not tilt or turned (portrait style). Head covering is not permitted except for religious reasons, but the facial features from bottom of chin to top of forehead and both edges of the face, with both ears must be clearly visible.

All columns in the Passport application form should be filled in neatly and in CAPITAL LETTERS. No column should be left blank.

2. Please append your signature at two places, one in the column provided in the first page of the application and second one on the second page, below Sl.No.26 (Under Declaration). Signature should match with that in their existing passport.

If the applicant wants to change the signature, he/she may do so. In that case, new signature should be

appended in the column provided in the first page of the application form. New as well as old signature

3. Clear copies of the relevant pages of the passport

4. Clear copy of the valid Permesso Di Soggiorno. If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should be attached.

REQUIREMENTS FOR APPLYING FOR RE-ISSUE OF PASSPORTS

CONSEQUENT ON CHANGE OF NAME

6. Gazette Notification or newspaper advertisement, in original. In the case of newspaper advertisement, intention of change of name should be published in two Indian newspapers (in the locality of the applicant’s residence in India) and the other one in Italy. Application can be submitted only after 30 days of publication of the advertisement (last published newspaper). Advertisement should be given by the applicant himself/herself, except in the case of minor children. In the latter case, advertisement can be given by their parents, but not by anybody else. While submitting application for new passport with the changed name, an Affidavit should also be submitted.

REQUIREMENTS FOR APPLYING FOR DUPLICATE PASSPORT IN LIEU OF

LOST/DAMAGED PASSPORT

1. APPLY ON-LINE at http://passport.gov.in/nri/OnlineRegistration.jsp?pocode and take a print out of the Passport application form. REQUIREMENTS FOR APPLYING FOR PASSPORT FOR ENDORSEMENT OF SPOUSE NAME

In case the marriage was solemnized in Comune, please bring original Marriage Certificate issued by the Comune, after getting it duly apostilled/attested from Prefuttura along with clear copy and also its English Translation (self attested).

In case the marriage was solemnized in India, please bring original Marriage Certificate issued by the competent authorities in India and a copy of the same.

REQUIREMENTS FOR APPLYING FOR RE-ISSUE (RENEWAL) OF PASSPORT IN CASE THE APPLICANT IS ON FAMILY VISA

REQUIREMENTS FOR APPLYING FOR RE-ISSUE (RENEWAL) OF PASSPORTS IN RESPECT OF CHILDREN BELOW THE AGE OF 18 YEARS,

8. Both the parents should append their signatures (signature should match with that in their passports) on the third pages of the application form.

REQUIREMENTS FOR APPLYING FOR PASSPORTS FOR NEW BORN CHILDREN

1. Register the Birth, on-line at : http://indiancitizenshiponline.nic.in/ic_form_public.aspx, within one year of the birth of the child. Upload the photograph of the child, signatures of both the parents, copies of the passports of both the parents, marriage certificate of the parents and birth certificate of the child.

2. If the birth of the child is not registered within one year of the birth, the reason for delay in registering the birth, in writing, duly signed by both the parents, is required. If the birth of the child is not registered within one year of the birth, the process will require longer time.

3. Take a print out of the completed form.

4. Parents should append their signature (signature should match with that in their Passports) at two places on the second page of the Birth Registration application (where it is written Signature of parents).

5. APPLY ON-LINE for passport at: http://passport.gov.in/nri/OnlineRegistration.jsp?pocode and take a print out of the Passport application form. Please note that since a new born baby cannot sign, his/her thumb impression (left hand in case of male child and right hand in case of female child) should be appended on the first page of the passport application form (space provided).

6. Both parents should append their signatures (signature should match with that in their passports) on Page No.2 of the application (Under Sl. No.26..DECLARATION) and also on the third page of the application form.

7. Copy of the birth certificate issued by Comune along with its English translation (self attested). The birth certificate should have the names of both the parents.

8. Clear copy of valid Permesso Di Soggiorno of the child,

9. Clear copies of all the relevant pages of the passports of both the parents

10. Clear copies of Permesso D’Soggiorno of both the parents. If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should be attached.

11. Clear copy of the Address proof( Carta D’Identita or any other proof like any Utility bill-electricity/water/fuel bill or rent agreement, Ospity and its translation in English) of the parents.

12. Copy of the Marriage Certificate of the parents if their names are not endorsed on each other’s passport.

REQUIREMENTS FOR CANCELLATION OF INDIAN PASSPORT CONSEQUENT ON ACQUIRING OF FOREIGN NATIONALITY AND FOR APPLYING FOR OCI CARD

Please bring original as well as clear copies of all documents, duly self-attested. Original documents will be returned immediately after verification.

Please submit the application with documents, keeping them in the following order:

INDIAN CITIZENS, WHO HAVE TAKEN FOREIGN CITIZENSHIP OR INDIAN CITIZENS BORN TO INDIAN PARENTS OR INDIAN ORIGIN PARENTS IN A FOREIGN COUNTRY OR THEIR GRANDPARENTS OR GREAT GRANDPARENTS WERE INDIANS OR INDIAN ORIGINS AND EVEN IF THEY HAVE NEVER HELD INDIAN PASSPORT ARE BASICALLY INDIAN ORIGIN ONLY.

THEY MAY PLEASE APPLY FOR OCI CARD UNDER NORMAL CATEGORY.

FOREIGN NATIONALS, WHO ARE ELIGIBLE FOR OCI CARD BY VIRTUE OF BEING MARRIED (MARRIAGE SHOULD HAVE BEEN EXISTING FOR A MINIMUM PERIOD OF TWO YEARS ) TO INDIANS OR INDIAN ORIGINS MAY PLEASE APPLY UNDER "FOREIGN SPOUSE" CATEGORY.

CANCELLATION OF INDIAN PASSPORTS IN CASE OF ADULTS

1. Application form, duly filled in and signed by the applicant (signature should match with that in the Indian passport).

2. One photograph to be affixed on the application form.

3. Copies of all the pages of the Indian Passport, including blank pages. Each page of the photocopy to be self attested.

4. Copy of the relevant page of Italian/Foreign passport (self-attested),

5. Copy of Carta D’Identita of the applicant (self-attested)

OCI CARD IN CASE OF ADULTS

1. APPLY ON-LINE at https://passport.gov.in/oci/capchaAction and upload the photograph and signature of the applicant.

2. Please upload the photograph, signature, copy of the Foreign Passport (one page where the photo, name, date of birth, etc appear), copy of the Indian passport (first page and last page) or the proof of being an Indian origin and marriage certificate, if the Indian passport does not have the spouse name endorsed in it.

3. Take the print out of the application form. Please paste one recent colour photograph of the size of 2 inch x 2 inch with light background in the space provided in the first page of the application. The photograph should have a frontal view, showing both the ears. One photograph may be attached with the application form.

4. Copies of the relevant page of the Italian/Foreign passport (self-attested),

5. Copies of the relevant pages of the Indian passport (page where the photo/name/date of birth, etc. appear, page where the names of parents and address appear)-self-attested

6. Copy of the Marriage Certificate – self-attested (if married) if spouse name is not endorsed on their Indian passports

CANCELLATION OF INDIAN PASSPORTS IN THE CASE OF CHILDREN BELOW THE AGE OF 18 YEARS

1. Application form, duly filled in and signed by the applicant. If the applicant is an infant and cannot append signature, thumb impression is required (left hand in case of male and right hand in case of female child).

2. Both the parents should also append their signatures (exactly as given in their Indian Passports or OCI cards) near to the place where the thumb impression of the child applicant is appended,

3. One passport size photograph of the applicant (to be pasted on the application form)

4. Copies of all the pages of the Indian Passport, including blank pages, with signature of the applicant’s father or mother, on all the pages(self-attested),

5. Copy of the relevant page of Italian/Foreign passport(self-attested),

6. Copy of Carta D’Identita of the applicant(self-attested),

7. Copies of the relevant pages of the passports/OCI cards (if any) of both the parents(self-attested)

8. Copies of the Carta D’Identita of both the parents(self-attested).

OCI CARD IN CASE OF IN CASE OF CHILDREN BELOW THE AGE OF 18 YEARS

1. APPLY ON-LINE at https://passport.gov.in/oci/capchaAction and upload the photograph and signature or thumb impression of the applicant.

2. Please upload the photograph, signature, copy of the Foreign Passport (one page where the photo, name, date of birth, etc appear), copy of the Indian passport (first page and last page) or the proof of being an Indian origin. In the case of children, who has never held an Indian passport, please upload the birth certificate (in English) and the copy of the Indian passport or OCI card of one of the parents.

3. Take the print out of the application form. Please paste one recent colour photograph of the size of 2 inch x 2 inch with light background in the space provided in the first page of the application. The photograph should have a frontal view, showing both the ears. One photograph may be attached with the application form

4. Both the parents should append their signatures on the second page of the application (signature should match with that in their Indian passports or OCI Cards),

5. Copies of the relevant pages of the Italian/Foreign passport of the applicant(self-attested),

6. Copies of the relevant pages of the Indian passport (page where the photo/name/date of birth, etc. appear, page where the names of parents and address appear) (self-attested)

7. Copy of Carta D’Identita, of the child(self-attested by either of the parents)

8. Copies of the relevant pages of the Indian passports/OCI cards/Italian or Foreign passports of both the parents(self-attested)

9. Copies of Carta D’Identita of both the parents (self-attested)

10. Copy of the Permesso Di Soggiorno of the parents(self-attested), if they are holders of Indian passports

11. Copy of the birth certificate(self-attested by parents) issued by Comune along with its translation in English (in case the child was not issued any Indian passport), duly attested by Prefutura.
